Meaning
Blood में Magnesium कम होना।

4️⃣ Bone Fracture Risk
क्यों?
लंबे समय तक Acid Suppression से Calcium Absorption पर कुछ प्रभाव पड़ सकता है, और कुछ अध्ययनों में Hip, Wrist एवं Spine Fracture के जोखिम में हल्की वृद्धि देखी गई है।
यह जोखिम विशेषकर—
Elderly PatientsOsteoporosis वाले मरीज
High-dose एवं Long-term Therapy
में अधिक महत्वपूर्ण हो सकता है।
5️⃣ Infections
Acid सामान्यतः कई सूक्ष्मजीवों के विरुद्ध एक प्राकृतिक सुरक्षा प्रदान करता है।
जब Acid लंबे समय तक कम रहता है—

1️⃣ Clopidogrel
क्या समस्या है?
Clopidogrel एक Prodrug है।
इसे Active बनने के लिए मुख्यतः CYP2C19 Enzyme की आवश्यकता होती है।
Omeprazole और Esomeprazole CYP2C19 को Inhibit कर सकते हैं।
इससे कुछ मरीजों में Clopidogrel की सक्रियता कम हो सकती है।
Clinical Point
यदि Clopidogrel आवश्यक हो, तो कई Guidelines में Pantoprazole को अपेक्षाकृत बेहतर विकल्प माना जाता है।
2️⃣ Warfarin
कुछ PPIs Warfarin के प्रभाव को बदल सकती हैं।
यदि दोनों दवाएँ साथ दी जाएँ—
INR Monitoring की आवश्यकता हो सकती है।
3️⃣ Ketoconazole / Itraconazole
इन दवाओं के Absorption के लिए Acidic Environment आवश्यक होता है।
यदि PPI दी जाए—
Acid कम होगा

इन Antifungal Drugs का Absorption कम हो सकता है।
4️⃣ Digoxin
कुछ मरीजों में Digoxin का Absorption बढ़ सकता है।
विशेषकर Elderly या Renal Disease वाले मरीजों में सावधानी रखी जाती है।
5️⃣ Methotrexate (High-dose)
High-dose Methotrexate के साथ कुछ PPIs Methotrexate Clearance को कम कर सकती हैं।
ऐसी स्थिति में चिकित्सकीय निगरानी आवश्यक है।
